The SR-22 is a form that the DMV requires so the DMV can track whether you pay for your insurance or let it lapse. The insurance company that insures your car(s) is required to send this form to the DMV if you request it. Your insurance company (if you are insured) cannot raise your rates or cancel you until you have finished your present term.
